L’anémie Hémolytique Auto Immune

Abstract

Le terme « anémie » se rapporte à une diminution du nombre de globules rouges et du taux d’hémoglobine dans le sang. Le terme « hémolytique » indique que la diminution des globules rouges est liée à leur destruction prématurée. Le terme « auto-immune » signifie que cette destruction est due à un anticorps produit par votre système immunitaire et dirigé contre vos propre globules rouges Le diagnostic des anémies hémolytiques auto-immunes est un diagnostic clinico-biologique. Le test de Coombs directe (TDA) est la pierre angulaire de l'enquête étiologique d'une anémie hémolytique. Il permet de caractériser le mécanisme de l'hémolyse: immunologique ou non immunologique. Une attention particulière doit être apportée à l'interprétation des résultats du TDA. Nous ferons la synthèse du diagnostic immuno-hématologique à entreprendre devant la suspicion d’une AHAI. Cette exploration biologique répond à un triple objectif : établir le diagnostic et le type du conflit auto-immun, détecter les facteurs prédictifs de sévérité et garantir la sécurité transfusionnelle. Nous proposerons à la fin un algorithme du diagnostic immunohématologies des AHAI.
